• Medical Form (2021)

  For your doctor to complete: Please have your doctor complete your Medical Form prior to attending any exercise class in U3A. If your appointment with your doctor is by phone, please ask the doctor to email you with approval for you to participate in the class/es in which you are enrolled. This will assist in case of a medical emergency.
